Enrollment Procedure

STEP 1: Download

Download the Online Enrollment Form. Fill out the editable Enrollment form completely, then save.

STEP 2: Email

Email the accomplished form to holyrosarycolleges1988@gmail.com.                    Please use the subject:    

GRADE LEVEL + COMPLETE NAME (e.g. GRADE 6 JUAN DELA CRUZ)

After sending the form, please wait for a submission confirmation.

For new students

Additional Admission Requirements

Kindly email the accomplished Enrollment Form – New Student, including a scanned copy of the following documents to holyrosarycolleges1988@gmail.com. 

  • 2×2 photo
  • Report card from previous school*
  • PSA-Issued Birth Certificate
  • Certificate of Good Moral Character*                                                                 

*if not yet available upon application, it may be submitted prior to school opening.    

STEP 3: Pay

Pay thru the Online BPI App or Over-The-Counter Deposit

STEP 4: Verify

Verify your payment. For Over-The-Counter payments and online transactions, please send a scanned copy of the deposit slip OR a screenshot of the successful payment via BPI app to:
